A woman in her 80s died and a man in his 20s lost his left leg after a small car struck him at a wharf in Shimonoseki, in Japan's Yamaguchi Prefecture, according to Japan's TBS NEWS DIG Powered by JNN.

The broadcaster reports that the collision happened around 6:15 a.m. on November 3. The man fell into the sea after being hit and was rescued, but suffered injuries requiring the amputation of his left leg.

The car also plunged into the water. Its driver was taken to a hospital, where she was confirmed dead, the report says. Footage shows the harbor water and police officers examining the wharf.

According to the report, police are considering whether the driver mistook the accelerator for the brake as they investigate the cause. That possibility has not been established as the explanation for the crash.
